Physical Fitness

I affirm that I am physically and medically capable of participating in HAFC activities and am not aware of any condition that would make my participation unsafe. I understand that football (soccer) is a physically demanding sport and involves the risk of injury.

Acknowledgment and Assumption of Risk

I understand that participation in indoor or outdoor football activities involves inherent risks, including collisions with other players, falls, contact with equipment or playing surfaces, weather conditions, and other foreseeable or unforeseeable hazards.

These risks may result in minor or serious injury, permanent disability, death, or damage to personal property. I knowingly and voluntarily assume the risks associated with my participation.

Release of Liability

To the fullest extent permitted by law, I release and agree not to sue Hudson Athletic FC, its organizers, officers, employees, volunteers, contractors, referees, participants, sponsors, affiliates, municipalities, recreation departments, field operators, property owners, landlords, and any other parties involved in providing facilities or services for HAFC activities (collectively, the “Releasees”) for claims arising from my participation in HAFC activities, including claims involving ordinary negligence where permitted by law.

This includes, when applicable, facilities or programs operated by Hudson County, Jersey City, Bayonne, and other municipalities or organizations whose premises are used for HAFC activities.

Indemnification

I agree to indemnify and hold harmless the Releasees from claims, damages, losses, liabilities, and reasonable legal expenses arising from my actions or participation in HAFC activities, except where prohibited by law or resulting from willful misconduct by a Releasee.

Medical Treatment

If I become injured or experience a medical emergency while participating in an HAFC activity, I authorize emergency medical assistance to be sought on my behalf when reasonably necessary.

I understand that HAFC does not provide medical services and is not responsible for the cost of medical treatment, transportation, or emergency services. Emergency services, including 911, may be contacted when appropriate.

Personal Property

I understand that HAFC is not responsible for the loss, theft, or damage of personal belongings brought to matches or other activities.

Participant Responsibility

I agree to follow reasonable rules and instructions provided by HAFC organizers, referees, facility staff, and other authorized personnel. I understand that unsafe, threatening, abusive, or excessively aggressive conduct may result in my removal from an activity or suspension from future HAFC events.

Acknowledgment

I have carefully read this waiver and release, understand its terms, and understand that by signing it I am giving up certain legal rights.

I sign this agreement knowingly, freely, and voluntarily.
